What is Transitioning Care?
This specialized care helps ease your loved one’s transition from a hospital stay or a rehabilitation facility to the home. Some of the services Comfort Keepers® can provide includes the following:
- Support the plan of care to aid the recovery process.
- 24-hour around-the-clock care or live-in care to ensure peace of mind for family and friends.
- Assistance with household activities and responsibilities, including meal preparation, light housekeeping, laundry, and running errands.
- Conduct a safety walk through to make sure the home is free of any hazards (remove loose cords, secure loose carpets, add extra lights).
- Incidental transportation for follow-up visits.
